Although you have probably considered this, it sounds like you are being reinfected from contaminated items of clothing, bath towels, work gloves, bed sheets, etc.  I found this to be a problem myself last year.  Tecnu can be used to wash clothing as well as on the skin.  At Gemplers you can purchase 32oz bottles of Tecnu for $15.  This should be ample to wash your clothing.

Or just throw your towels and clothing away!
